Overview

Phenibut is a synthetic compound developed in Russia that acts on GABA receptors — the same system affected by alcohol and benzodiazepines. It is sold in the U.S. as an unregulated dietary supplement, marketed for anxiety relief, sleep improvement, and cognitive enhancement. It is not FDA-approved for any medical use.

Phenibut produces anti-anxiety and sedative effects similar to benzodiazepines and alcohol. Regular use can rapidly lead to tolerance and physical dependence. Phenibut withdrawal can be severe and dangerous, with symptoms including severe anxiety, insomnia, psychosis, tremors, and seizures — similar to benzodiazepine or alcohol withdrawal. Medical supervision during discontinuation is important.

Treatment

Treatment approaches mirror those for benzodiazepine dependence: medically supervised taper (often using gabapentin, baclofen, or benzodiazepines under medical guidance), behavioral therapy, and treatment of underlying anxiety or sleep disorders with safer alternatives.
