Overview

Tianeptine is an atypical antidepressant that is approved as a prescription medication in some countries but is not FDA-approved in the United States. Despite this, it is widely sold online and in gas stations, convenience stores, and smoke shops under brand names including ZaZa, Tianna, TD Red, and Pegasus.

At doses higher than therapeutic levels, tianeptine activates opioid receptors, producing euphoria and pain relief similar to traditional opioids. This has led to widespread misuse and a growing addiction crisis, particularly in Southern states. Tianeptine withdrawal closely resembles opioid withdrawal and can be extremely uncomfortable.

Several states have banned or restricted tianeptine, including Alabama, Michigan, Indiana, Georgia, Ohio, Tennessee, and others — but it remains legal in many jurisdictions.

Signs and Treatment

Signs of addiction mirror opioid addiction: escalating doses, withdrawal symptoms between doses, continued use despite negative consequences. Treatment follows opioid addiction protocols — medical detox, potential use of buprenorphine or other MOUD (on a case-by-case basis with provider guidance), behavioral therapy, and support groups.
